Date: 2017-03-01 02:53 am (UTC)It helps a lot that the root cause (as determined in the post-incident analysis) was an interaction between a simple-to-fix and small problem, and a whole lot of trauma (much less simple to fix). And the compounding factors also have some simple steps to help future events.
The way that everyone managed to have a completely lousy day is unfortunate, but not actually the fault of anyone but the jackasses who left all involved parties with massive amounts of trauma.
